3. Companies house certificate and constitution of the company in UK (Legalize by Minister Affairs in United kingdom FCO (www.fco.gov.uk/legalisation)
4. Bank statements (last 6 months) 
5. Flights details
6. Yellow Fever Certificate of Vaccination 
7. Valid passport with a minimum validity of 6 months 
8. Two passport size photographs (3.5 x 4.5 cm)
9. Police Certificate Original and Translation (both documents previously legalized by the FCO) (http://www.acro.police.uk/police_certificates.aspx)
   • *Send the above requirements/paperwork on .pdf format to the Consulate email address: consulate@bolivianembassy.co.uk; requesting an appointment for the Visa Application
10. Fees – pay at the consulate by online transfer once you are at the consulate; fees will be confirmed at the appointment.
